The order process of care
begins with a phone call to Boca Home Health at 561-862-6072
by a family member, friend, physician or other health care
professionals. The Boca Home Health intake coordinator will ask general
information, including permission to contact the patient and the primary
care physician to ensure continuum of care.
A time will be arranged for one of our professional registered nurses to assess, evaluate and consult with the patient, the family and the primary care physician. A registered Nurse will develop and implement a personalized home care plan that addresses the client's health care needs, values and preferences.
The patient's primary nurse will monitor and coordinate all aspects of a client's care, schedule physician visits/special care services, perform regular health assessments, help with administering prescribed medication and provide 24/7 on call (RN) registered nurse assistance.
Boca Home Health supports residents throughout Palm Beach, Indian River, Martin, Okeechobee and St. Lucie counties.
